Participant personal information may be collected from:
- you, the participant
- your family members or significant others
- your advocate
- your doctor or other service providers or facilities.
We will collect personal information directly from you unless
- we have your consent to collect the information from someone else
- we are required or authorised by law to collect the information from someone else
- it is unreasonable or impractical to do so.
You can withdraw your consent at any time by contacting us; although you should be advised that this may impact on our capacity to provide services.
Purpose of collecting personal information
Personal information is collected for the purposes of providing care and services. The information may be used to:
- provide support services
- enable service providers and medical practitioners to provide care and services
Disclosure of personal information
We may disclose your personal and health information, for the purpose of your care and services, to:
- Service providers, medical practitioners
- External health agencies, NDIS, and government organisations
- Your nominated advocate or emergency contact
- We limit disclosure to purposes related to care, except where consent is given or in exceptional circumstances outlined by law.
We will not disclose your personal information to an overseas recipient.
Data Security
We are committed to safeguarding your information against misuse, loss, or unauthorized access. Our security measures include secure databases and cloud-based technology accessible only to authorized staff.
Accessing Your Information
Under the Privacy Act, you have the right to access your information. To request access or amend your information, please contact us. Identification proof is required, and we aim to respond within seven days.
Employee and Volunteer Information
Employee and volunteer records are managed in line with workplace and privacy laws. Employee information is subject to privacy laws when used outside of the employment context.

In case of a data breach, our Management of Data Breach Policy and Procedure will be implemented to address the issue effectively.
Privacy Complaints
We take privacy complaints seriously and handle them promptly and confidentially. Our complaint handling process ensures no impact on your existing obligations or commercial arrangements with us. You will be informed of the outcome following an investigation.
